[Rhinoplasty] long / Arrow Nose Surgery Korea

The nose, positioned at the center of the face, is considered a crucial part that determines a person's impression. As the nose is a three-dimensional feature among facial attributes, its overall image can change based on the shape of the bridge and the tip. In reality, if the proportions of the nose are well-matched and harmonious, it tends to convey a refined and attractive image, however, nose shapes where the tip droops below the overall facial proportions, making the nose look long, or conversely, too short resembling a pig's nose, can disrupt the overall balance of the face, leading to a less sophisticated image. Especially if the nose is long compared to the length of the face, or the tip droops, giving an Arrow Nose appearance, it can make one look older, impart a severe and strong impression, leading to a complex about one's appearance.

Typically, an Arrow Nose is characterized by the tip pointing downwards, obscuring the philtrum between the nose and the lips. This happens when the tip is lower and longer than the nasal bridge, giving the appearance of an arrow tip pointing down, making the nose look longer, hence it is also referred to as a Long Nose. The cause of such an Arrow Nose is often due to the improper placement of the alar cartilage and the overdevelopment of the septal cartilage compared to the norm. When the muscle that pulls down the tip of the nose, the depressor septi nasi muscle, acts excessively, it can cause the nose tip to droop when smiling, resulting in an Arrow Nose appearance, which can be corrected surgically.

Arrow Nose surgery involves the process of trimming the drooping nasal tip cartilage, repositioning it upwards, securing it, and supporting it with autologous cartilage struts to prevent it from drooping again. During this process, it's not merely about moving the tip of the nose, but selecting an angle and height that harmonize with the individual’s facial features, extending from the nasal bridge to the tip and jawline, to achieve a highly satisfactory outcome.

For example, if the septal cartilage has grown excessively, part of the lower section should be removed. Then, the tip cartilage is repositioned and secured higher up. If the alar cartilage is large and drooping, making the nose appear like an Arrow Nose, it needs to be rotated upwards, repositioned, and fixed to the septal cartilage. Additionally, if an Arrow Nose has worsened due to the development of the nasal tip muscles, it is advisable to perform a partial excision of the muscles that pull the tip downwards to prevent the tip from drooping when smiling.

Furthermore, those with an Arrow Nose are likely to have a deviated septum, used as a strut in cases like a hump nose. In such instances, it is necessary to correct this to improve the functional aspects and alleviate issues like nasal congestion.

Thus, a Rhinoplasty for an Arrow Nose should take into account other factors such as the degree of nasal droop, skin thickness, and nostril size. The nose, being central to the face and significantly influencing appearance, should be modified to fit facial proportions rather than aiming for a nose that is high and ornate. Hence, it is crucial to consult with a medical team that has ample experience and surgical skill, to undergo a detailed analysis and diagnosis of the nasal issues with tools like 3D CT, and to establish a meticulous surgical plan.

Therefore, it is advisable to visit a facility that offers individualized consultations with clinically experienced medical staff, post-operative care for recovery, and a thorough safety system, to receive accurate diagnoses and create an ideal nose line that harmonizes with the face.
