When we are thinking about double eyelid surgery, we use the expression "poking the eyes" a lot. This is usually used when we use non-incisional method to make a double eyelid line. Especially for those who think incision is burdensome, they tend to prefer non-incisional method that can create double eyelid lines using thread. I think this is because the recovery time of non-incisional method is short, and vivid and clear eyes can be created naturally without incision. 

However, you need to know that the approach method of double eyelid surgery needs to be different depending on the characteristics of the double eyelids. If there are many fats inside the thick eyelid, or the eyelids are thick and the sagging problem of the skin is serious, we need to burn out or remove the fats through incision method. On the other hand, if the skin is kind of thin while there are many fats, partial incision is a good method to create a desirable double eyelid line. And non-incisional double eyelid surgery is an appropriate method for people whose skin is thin while there are only little sagging and no soft tissues, who think that incision is too burdensome. After considering the image and anatomical characteristics which are different from person to person and decided a design line, a fine thread is used through 3-5 small holes on the upper eyelid that lines along the design line, in order to tie the knots firmly without letting the double eyelids be loosened. This method is called non-incisional method. As there is no incision, and the tissue damage is being minimized, there is almost no swelling, and in addition, the recovery is quick and the burden of scarring is smaller compared to incision method, so it is gaining popularity steadily. However, it does not mean that there will be 100% no scars if we do the surgery in non-incisional method. In general, when we use non-incisional method to do the double eyelid surgery, there will be needle holes formed on the double eyelid line, and as they are very fine needle holes, we do not need to sew them up. It is different from person to person that some people may got reddish skin and pigmentation like brown color for a while, so some people may be worried about that, but the scars will become totally unobvious  in a few months. 

For non-incisional method, although there are rarely bruises (internal bleeding) on the surgical area, depending on the individuals, in some rare cases, if the needle touches the blood vessels during anesthesia, internal bleeding may happen. Even if there are bruises, they will be absorbed in 1-2weeks and disappear without any traces. 

One of the question that many of the people who are considering having double eyelid surgery and visited our hospital ask is that how much swelling will they get if they do the double eyelid surgery in non-incisional method, and how long will the swelling period be if they want to obtain an natural eyes. Normally, the swelling of non-incisional double eyelid surgery is the most serious in 2-3 days after the surgery, (although it will be different depending on the individuals' eye conditions) and after 1-2 weeks, the large swelling will disappear gradually. In the condition when there is swelling after the surgery, although the width of the double eyelid will look wide, if the swelling goes down in a certain extent, the natural line that we first designed will be completed gradually. In the other words, starting from 1 month after the surgery, you will be able to obtain a pair of eyes with natural double eyelids, which does not look like a surgery has been done. There are people who get stressed when their eyelids are unevenly swollen. That is a very natural phenomenon, so you do not need to worry about that. Since everyone is not symmetrical, same for the swelling of the eyelids, there can be difference. If the swelling goes down, a natural and well defined double eyelids will be completed. But if a few months have passed after the surgery, and your eyes are still look very asymmetrical, or they look very unnatural , it is desirable for you to visit the hospital and have a consultation with the doctors who performed your surgery. 

If you still want to get rid of the swelling quickly, it is recommended that you can use cold pack after the surgery. After using non-incisional method, there will be a little heat on the surgical area, so it is good for you to use ice pack to cool it down for 2-3 days. At this point, rather than put the ice pack on the eyelids directly, it is better to cool the areas close to the eyelids, like the eyebrows or temples. Also, if you sleep with your head higher than your heart after surgery, it is one of the ways to suppress swelling as it reduces blood circulation in your eyelids. Moreover, it is recommended to avoid extreme exercise, bathing and smoking, for about 1-2 weeks after surgery. If you want to do exercise, you can take a light walk, or if you need to take bath, you can take a very short bath or take shower to replace bathing . These acts can help to reduce the swelling faster. 

However, the swelling management after the surgery does not affect the double eyelid line very much. Instead of swelling management, the more important thing is that you should find a place which is able to fulfill your needs, and have sufficient consultation with medical team who have extensive experience.
